Medical Therapies
Various hormonal therapies can significantly improve bleeding problems. Oral contraceptives provide not only a method of birth control, but they are often the first line of therapy for patients with heavy bleeding, irregular periods, recurrent ovarian cysts or severe menstrual cramps. More recently, a progestin-containing intrauterine device (IUD) has been made available. Much like an oral contraceptive, it prevents pregnancy, but the hormone imbedded in the IUD has a local effect on the lining of the uterus that can significantly reduce menstrual flow and cramping as well. For a subset of patients with polycystic ovarian condition (PCOD), the use of insulin-lowering agents (often used to treat adult-onset diabetes) has been shown to regulate (even notoriously irregular) menstrual cycles. Non-steroidal anti-inflammatory medications remain the mainstay of therapy for heavy, painful periods. Various combinations of the above therapies can be tailored to each patient’s needs.
